Delia, Diane (1957 – 7 October 1981) delusion of sexual transformation Extremely rare type of delusion, sometimes referred to as a delusional misidentification syndrome, in which an individual believes that their body, or the bodies of others, have spontaneously transformed into that of a different gender-related phenotype. For example, a person who identifies as male and has a penis and testicles and lacks breasts believes that they have spontaneously grown breasts and a vagina and have lost their penis and testicles. Such occurrences appear to be linked to seizures or, occasionally, to schizophrenia, but their mechanism is unknown. It should be noted that such delusions are entirely unconnected to being transgender. A transgender person almost always recognizes their current phenotype and seeks out gender-affirming care to change that phenotype; those experiencing sexual transformation delusions believe that their phenotype has already changed or is changing for any of various reasons related to the delusion.
